John G. Hubbard, MD
Dr. Hubbard is a Kentucky native, born and raised in Munfordville. He attended Centre College, and graduated from
University of Louisville School of Medicine, as an elected member of Alpha Omega Alpha honor medical society. He received post-graduate training at UofL and at Emory University in Atlanta, and is a Board Certified Urologist. Dr. Hubbard has been practicing Urology since 1978. He has been a solo practitioner since 2000 and his assistant, Sandy, has worked with him for 20 years. Dr. Hubbard has four children with his wife of 44 years.

Emmy H. Baker, RN, MSN, APRN
Emmy Baker is a Louisville native who received her undergraduate degree from Miami University in Oxford, Ohio and continued on to receive her Master's degree in Nursing from Vanderbilt University. She has a dual degree as a Women's Health and Adult Health Nurse Practitioner. Emmy has been with the Hubbard Clinic since 2008. She is married with one two young children. Patricia Hooker, PA-C 
Patty Hooker is a physician assistant who graduated Summa Cum Laude from the Medical University of South Carolina in 2003. She received her undergraduate degree at Wittenberg University in Springfield, Ohio. She has been a board certified PA for 9 years and has been with the Hubbard Clinic since 2010. Patty is married with two young daughters.
