All women will see our female nurse practitioners for a physical exam and then will see Dr. Hubbard immediately following for treatment discussion and planning.

Please be sure to bring your insurance cards and referral, if needed.

To obtain a referral, you will need to contact your family physician at least 72 hours in advance.

Please arrive 30 minutes early for your appointment time with completed paperwork in hand.
